office visits or clinical appointments following Dr. Stupi’s retirement from direct patient care.

provided and managed by Horizon Infusions.​​

 

Medical Records

Effective August 1, 2026, Cariend will securely manage patient records for seven (7) years.

Cariend is not a healthcare provider and cannot assist with billing, prescriptions, referrals,

or other medical services.

To request records, submit a completed Records Release Authorization Form and a copy of a

government-issued photo ID, as required by HIPAA.
